Appointment Outcome Question Customization

Create customization available for appointment outcomes to be limited to 1, 2 or all 3 questions.
Specifically useful for a team who uses limited stages and once an appointment is set the lead goes to "Active Client" stage - there is no need for the second questions of stage update as they are already in the correct stage of "Active Client."
